Incident Registration Form

The reports are recorded in a uniform manner, so they can be compared with other reports. This is the way to learn from other associations and they from us. Therefore, when reporting an incidence, use the Incidence Registration Form.

    This is not mandatory. Being aware of the reporter can help the safety committee to ask additional questions. After the investigation has been completed, the identifying data will always be removed from the form.
